Demi Moore was rushed to the hospital Monday night, reports TMZ. The actress is allegedly being treated for exhaustion and substance abuse issues. According to Us Weekly, Moore has bowed out of the film about porn star Linda Lovelace , which stars Amanda Seyfried in the title role.

Moore was hospitalized Monday night after a 911 call. According to TMZ, she is being treated for substance abuse issues. Her representative termed it exhaustion.

Despite the substance abuse claims, Demi's rep says of his client: "Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health.

"She looks forward to getting well and is grateful for the support of family and friends."

Moore, 49, who announced she checked into treatment for exhaustion, has not commented on the claims.

Sources close to both Moore and Kutcher tell PEOPLE that Moore's prescription drug use was a contributing factor to the end of her marriage to Kutcher, 33.

"It was a sticking point for Ashton," a source tells PEOPLE. "He wanted her to take care of herself and get a hold of things, and she wouldn't."

Adds the source, "Her family and friends have been really worried."
